Permit Submittal Requirements for the City of Hollywood:

BROWARD COUNTY ENVIRONMENTAL PROTECTION DIVISION
Broward County EPD Approval required prior to permit issuance, when applicable.

ELEVATOR

Broward County Elevator submittal / Approval required prior to Bldg Dept approval, when applicable.

NOTICE OF COMMENCEMENT

Recorded NOC submitted to the Bldg Dept and posted on job site prior to first inspections if job cost exceeds $2,500 or A/C repair or replace $7,500 or more.  The NOC is not required for permit issuance but is required to be able to schedule the first inspection.

PERMIT APPLICATION

Permit applications must be completed in their entirety and must be signed byu Property Owner/Agent and Contractor.  Proof of Lic and Insur is required. If value of the work is less that $2,500, only the contractor needs to sign.

SURVEYS
Current s/s survey and elevation certificate required for new buildings, structures, additions and site work.  An s/s elevation certification is required on any work that might be a substantial improvement, interior or exterior.  Spot s/s survey required before first tie beam inspections with noted slab elevations of new buildings or additions.  Final s/s survey and elevations certificate required prior to final of new buildings, structures, additions and site work. .

ENERGY CALCULATIONS
2 sets when required

SPECIAL INSPECTION FORMS
Must be s/s by special inspector and must be signed by Property Owner and the Contractor.

STRUCTURAL CALCULATIONS

Two copies must be s/s by engineer of record.

ORIGINAL DEVELOPMENT REVIEW COMMITTEE PLANS

Projects must go through a Pre-application Conceptual Overview (PACO) and then on to the Technical Advisory Committee (TAC) prior to applying for Building Permit.

OWNER/AGENT LETTER
Notarized affidavit required from Property Owner authorizing Agent to be able to sign on behalf of Property Owner.

NUMBER OF SETS
Two sets.

TRADE APPLICATIONS
Permit applications must be completed in their entirety and must be signed by Property Owner/Agent & Contractor. Proof of License & Insurance is required. If value of work is less than $2,500, only the contractor needs to sign

NOTICE OF ACCEPTANCE

All NOA's must be reviewed and approved by the design professional of record if they are part of a designed set of plans. If the NOA's are for replacement permits which do not require professionally designed plan, then they are not required to have the approval.

ROOF CALCULATIONS (HVC)
Must provide two completed HVHZ uniform roofing permit applications and attachments as required.

ROOF TRUSS SHOP DRAWINGS
Truss package (engineered truss design and layout must be s/s by truss engineer and reviewed and approved by the designer of record.

FIRE SPRINKLER ALARM DRAWINGS
Must have signed and sealed sprinkler drawings and hydraulic calculations. Cut sheets required for all components used.

FLOOD CERTIFICATIONS
Certificate required for Substantial improvements or Damage determination and prior to final for compliance with project plans of new buildings and additional.  Floodproofing certificate required when floodproofing non-residential new construction, substantial improvements and additions. V-Zone Certificate required for V-Zone new construction, substantial improvements and additions.  No documents required for only interior non-substantial improvements

LANDSCAPING REQUIREMENTS
Tree survey and appraisal for existing trees onsite.

ENGINEERING REQUIREMENTS
Any work done in right of way requires a separate permit application.

COMMENTS
REVISIONS: Need Transmittal Form. Need Architect's Narrative. Need clouds and deltas. Use Transmittal Form for 'Corrections'. Signatures Not required.
