South Florida Community Care Network Rebrands As Community Care Plan

The health plan with a heart that serves Medicaid, Children’s Medical Services Network, and Self-insured Employee Plans

Sunrise, Fla. (July 21, 2016) – South-Florida-based South Florida Community Care Network (SFCCN), is pleased to announce that it has changed its name to Community Care Plan and is in the process of introducing the new brand. Introduced in 2000 as the first safety-net, hospital-owned Provider Service Network (PSN), Community Care Plan: the health plan with a heart, serves members enrolled in Medicaid, Children’s Medical Services Network and self-insured employee health plans. Headquartered in Sunrise, Florida, Community Care Plan is a partnership owned by Broward Health and Memorial Healthcare System. Community Care Plan is led by healthcare industry veteran John A. Benz, President & CEO of the company. 

The non-profit organization is rebranding its name to Community Care Plan to better emphasize its mission to positively impact the health and welfare of those that the company serves. Community Care Plan’s health plans cover a wide range of health and medical services and offer an excellent choice of physicians and other benefits that help enrollees get and stay healthy. Community Care Plan is fully accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) as a Health Plan and is currently the only remaining Provider Service Network in the state of Florida.

Community Care Plan currently serves over 100,000 individuals and families through several health plans:

  • One of four Medicaid plans serving Broward County residents.
  • Partnered with the Department of Health/Children’s Medical Services (CMS) to form an Integrated Care System (ICS) to administer the benefits of children with complex medical needs.
  • Administers self-insured governmental employee health plans, including Memorial Healthcare employee plans.
  • Administers uninsured/charity programs. 

In order to achieve excellence in serving the community, Community Care Plan’s Six Core Principles of Excellence (quality, customer service, community, people, finance and growth) represent the foundation of its vision to transform the healthcare experience.  The organization prides itself on providing a Collaborative Care Coordination team approach and delivering quality service to its members and providers.
